PASSPORT RENEWAL CHECKLIST

The documents listed below are required for a renewal of a U.S. passport:

• American passport (current or expired)

• Birth certificate (original or certified copy)

• If minor child: passport or ID of mother & father, both parents need to be present

•  If minor child & one parent not present: consent form filled out with copy of parent's ID/w signature (passport or drivers license etc.) 

• Pictures of child growing up from age in passport to present age

• One 2x2 passport size picture, can be in color but the background must be white                        

• If minor child & parents are divorced: a copy of divorce decree

• If minor child & one parent is deceased: a copy of the death certificate

• If minor child & guardian has child, a letter of permission by the parents giving permission to obtain a renewal passport.                                     

• Passport fee for children under 16 years of age $82.00

• Passport fee for children/adults over 16 years of age $97.00
