PASSPORT RENEWAL CHECKLIST

The documents listed below are required for a renewal of a U.S. passport:

 Passport application form.

 American passport (current or expired).

 Birth certificate (original or certified copy).

 One 2x2 passport size picture, can be in color but the background must be white.

 Passport fee for children/adults over 16 years of age $100.00
 (cash only and no pre-1997 bills).

 American citizen hours are 1.30-3.30 Monday-Thursday.

Specifically for minors:

 Passport or ID of mother & father, both parents need to be present.

 If one parent not present: consent form filled out with copy of parent’s ID w/signature
(passport or drivers license etc).

 Pictures of child growing up from age in passport to present age.

 If parents are divorced: a copy of divorce decree.

 If one parent is deceased: a copy of the death certificate

 If in custody of guardian, a letter of permission by the parents giving permission to obtain a renewal passport.

 Passport fee for children under 16 years of age $85.00
 (cash only and no pre-1997 bills)
